MORTGAGE APPLICATION CHECKLIST

PROPERTY INFORMATION
	Copy of Purchase and Sales Agreement (if purchase)
	Copy of the down payment check (if purchase)
	Name and phone number of all real estate agents involved in the transaction (if purchase)
	Description of the property to include real estate tax amount and year it was built

INCOME INFORMATION
	Name, address and telephone numbers of your employers for the past 2 years
	Students or recent graduates without a 2 year work history need diploma and transcripts
	Copy of pay stubs to cover a period of one month; must include year to date income.
	Copy of the past two years W-2 forms
	First Time Home Buyers to provide copy of the last 3 years signed personal tax returns
	If using income from alimony or child support a copy of the divorce decree
	Self-Employed Borrowers – Please provide:
	Copy of the last 2 years signed business and personal tax returns with all schedules
	Year to date Profit and Loss Statement
	Copy of business license and CPA letter verifying 2 years self-employment
	Rental Property Income verified by signed lease agreement and federal tax returns

ASSET INFORMATION
	Copy of 2 month bank statement on all deposit accounts
	Copy of retirement accounts / 401 K statements
	If the down payment is a gift a signed gift letter along with verification of the gift funds
	Schedule of any other real estate you may own

LIABILITIES
	Names of all creditors along with monthly payments and balances

OTHER INFORMATION
	Social Security number required for all borrowers
	Date of Birth required for all borrowers
	Attorney information to include; name, address and phone number
